One of the most recent controversies to arise in the field of bioethics concerns the ethics for the Groningen Protocol: the guidelines proposed by the Groningen Academic Hospital in The Netherlands, which would permit doctors to actively euthanise terminally ill infants who are suffering. The Groningen Protocol has been met with an intense amount of criticism, some even calling it a relapse into a Hitleresque style of eugenics, where people with disabilities are killed solely because of their handicaps. The purpose (...) of this paper is threefold. First, the paper will attempt to disabuse readers of this erroneous understanding of the Groningen Protocol by showing how such a policy does not aim at making quality-of-life judgements, given that it restricts euthanasia to suffering and terminally ill infants. Second, the paper illustrates that what the Groningen Protocol proposes to do is both ethical and also the most humane alternative for these suffering and dying infants. Lastly, responses are given to some of the worries expressed by ethicists on the practice of any type of non-voluntary active euthanasia. (shrink)

Objectives: to assess physicians’ and patients’ views in Saudi Arabia towards involving the patient versus the family in the process of diagnosis disclosure and decision-making, and to compare them with views from the USA and Japan.Design: A self-completion questionnaire was translated to Arabic and validated.Participants: Physicians from different specialties and ranks and patients in a hospital or attending outpatient clinics from 6 different regions in KSA.Results: In the case of a patient with incurable cancer, 67% of doctors and 51% of (...) patients indicated that they would inform the patient in preference to the family of the diagnosis . Assuming the family already knew, 56% of doctors and 49% of patients would tell the patient even if family objected . However, in the case of HIV infection, 59% of physicians and 81% of patients would inform the family about HIV status without the patient’s consent . With regards to withholding ventilatory support, about 50% of doctors and over 60% of patients supported the use of mechanical ventilation in a patient with advanced cancer, regardless of the wishes of the patient or the family. Finally, the majority of doctors and patients were against assisted suicide.Conclusions: Although there was more recognition for a patient’s autonomy amongst physicians, most patients preferred a family centred model of care. Views towards information disclosure were midway between those of the USA and Japan. Distinctively, however, decisions regarding life prolonging therapy and assisted suicide were not influenced to a great extent by wishes of the patient or family, but more likely by religious beliefs. In our time, a time of fundamental societal changes associated with the development of the world socialist system, which conditions the progressive course of mankind's social development, the problem of the prevention of war has come to be of immense importance. This problem has not only the greatest practical significance, but also a theoretical, philosophical aspect. The philosophical aspect of this problem is reflected, in the first place, in the fact that some hold the view that a new world war (...) is predestined by history, and represents a fatal inevitability, so that struggle against it is "Utopian." This view denies the possibility of peaceful coexistence among the given countries differing in societal and political systems. Such a position is in irreconcilable contradiction with the teachings of Marxism-Leninism, which stand opposed to the bourgeois conceptions of Karl Jaspers, W. Schmidt, Raymond Aron, G. Wetter, and many others who employ pseudoscientific argumentation to propagate the idea that such peaceful coexistence is impossible. (shrink)

The Groningen protocol allows for the euthanasia of severely ill newborns with a hopeless prognosis and unbearable suffering. We understand the impetus for such a protocol but have moral and ethical concerns with it. Advocates for euthanasia in adults have relied on the concept of human autonomy, which is lacking in the case of infants. In addition, biases can potentially influence the decision making of both parents and physicians. It is also very difficult to weigh the element of quality of (...) life on the will to live. We feel an important line has been crossed if the international medical community consents to the active euthanasia of severely ill infants and are concerned about the extension of the policy to other at risk groups. The correctness of application of orientalist discourse of E. Said to the colonial policy of the Russian Empire is analyzed in the article on the example of P. I. Rychkov research. By studying integration of Bashkirs in the structure of the Russian state, the author came to the conclusion that Russia’s policy in the East was based on the experience of the management of non-Russian peoples, which was developed in the 16-17th centuries. The establishment of ‘cultural distance‘ is typical only (...) for a small group of educated administrators of past-Petrine era. Most of the officials of the region, as in the preceding period, and during the time of Rychkov and Tatishchev preferred to rely on traditional methods of Bashkir management, which was developed in the period of their voluntary accession. Managerial stereotypes of 16-17th centuries, had significant influence on policy towards South-Eastern peoples of Russia even in the 19th century. The Russian administration inherent in the desire to integrate the non-Russian peoples in the General class structure of the state on the basis of analogies or correspondences, the objective of which was social and cultural rapprochement of the two societies. The validity of the double effect doctrine is examined in euthanasia and abortion. In these two situations killing is a method of treatment. It is argued that the doctrine cannot apply to the care of the dying. Firstly, doctors are obliged to harm patients in order to do good to them. Secondly, patients should make their own value judgments about being mutilated or killed. Thirdly, there is little intuitive moral difference between direct and indirect killing. Nor can the doctrine apply (...) to abortion. Doctors kill fetuses as a means of treating the mother. They also kill them as an inevitable side effect of other treatment. Drawing a moral distinction between the direct and the indirect killing gives counterintuitive results. It is suggested that pragmatic rules, not ethics, govern practices around euthanasia and cause it to be more restricted than abortion. (shrink)

This paper distinguishes four major types of futility (physiological, imminent demise, lethal condition, and qualitative) that have been advocated in the literature either in a patient dependent or a patient independent fashion. It proposes five criteria (precision, prospective, social acceptability, significant number, and non-agreement) that any definition of futility must satisfy if it is to serve as the basis for unilaterally limiting futile care. It then argues that none of the definitions that have been advocated meet the criteria, primarily because (...) their proponents have not paid sufficient attention to the problematic nature of the data supporting the use of their definitions.
